From Incident to Insight: Investigation and Risk Assessment in Practice
1Summary
Most workplace incidents do not happen because of one dramatic failure — they happen because a chain of small warning signs went unnoticed or unaddressed. This course trains participants to break that chain: to investigate incidents thoroughly enough to find the real root cause, and to assess risk well enough to catch the next problem before it becomes an incident at all.
Combining theory, case studies, and hands-on exercises, the course walks participants through the full investigation cycle — from evidence collection and interviews to writing findings that lead to real corrective action — alongside a structured approach to hazard identification and risk mitigation. The result is a safety practice built on evidence and prevention, not paperwork after the fact.
